If you need to return your merchandise for any reason, please complete the Return Form section on the front of your packing slip. Then repack the product in the original packing materials used to ship it to you, and enclose your completed Return Form. Note: This form is for returns within the U.S., Puerto Rico, and other U.S. territories only. For your convenience, the Return Form includes a Return Label that you may use when shipping the package back to us. Simply attach the label to the outside of your package and send the package via UPS or insured parcel post. Please first remove the old label from the packaging or make sure that the returns label covers the old one.
